Onze huisartsenpraktijk is momenteel gesloten voor nieuwe inschrijvingen.
Wij maken een uitzondering voor patiënten die nog geen huisarts hebben in de wijk en woonachtig zijn in een van de volgende postcodegebieden: 3014, 3015, 3021, 3022 of 3023.
Voldoet u aan deze voorwaarden? Dan kunt u zich op twee manieren aanmelden:
1. Online aanmelden
Via deze link kunt u het inschrijfformulier online invullen. Wij beoordelen uw aanmelding en nemen indien nodig contact met u op. U hoeft hiervoor niet naar de praktijk te komen.
2. Aanmelden via de praktijk
U kunt bij de praktijk een inschrijfformulier ophalen. Vul het formulier volledig in en lever het daarna in bij de assistente op de 2e etage.
Neem bij inschrijving een geldig identiteitsbewijs mee. Zonder geldig identiteitsbewijs kunnen wij uw inschrijving niet volledig afronden.
Na bevestiging van uw inschrijving vragen wij u uw vorige huisarts te laten weten dat u bent overgestapt. Uw vorige huisarts kan uw medisch dossier vervolgens elektronisch aan ons overdragen.
Registration
Our GP practice is currently closed to new patient registrations.
We make an exception for patients who do not yet have a GP in the neighbourhood and live in one of the following postcode areas: 3014, 3015, 3021, 3022 or 3023.
If you meet these conditions, you can apply for registration in one of two ways:
1. Apply online
You can complete the online registration form via this link. We will review your application and contact you if necessary. You do not need to visit the practice to apply.
2. Apply at the practice
You can collect a registration form from our practice. Please complete the form and return it to the medical assistant on the 2nd floor.
Please bring a valid form of identification when registering. Without valid identification, we cannot fully complete your registration.
Once your registration has been confirmed, please inform your previous GP that you have changed practices. Your previous GP can then transfer your medical records to us electronically.
